I have a very strange bug on my App Drawer with the latest update. I normally arrange my apps into various folders. However, some of the folders randomly disappear and then the apps that should be in the disappeared folder return to the main page showing the Android icon rather than the app icon. Never had this issue before and have no idea how to fix it.

Do you try doing refresh from 3 dots menu?
Yes I had tried that. Even tried restarting phone a couple of times. In the end I had to add the apps to folders a second time. That fixed the missing icon for most but for some I had to manually change the icon under the select icon option.
Seen something similar on my old nexus 5 although not on the same scale. Similar issue with apps jumping out of folders but in my case only one shortcut was doing it. I’ve not seen this yet on my galaxy edge though.

Default android icons usually means the app is out of memory. Usually out of memory is itself a consequence of too much images (too many apps, large backgrounds, wallpapers, etc.)
