I had set it up similar to this but with a 3×9 grid. I was dragging the folders into columns and they started to randomly dissappear.
Could the speed of the drag and release have any bearing on whether the icons dissappear as I was rearranging them quite fast? If I released the folder icon before it had registered the position?

I don’t know, if this was a problem like this then I think a refresh should have been enough. My best guess at the moment is that the app “think” that the item is dropped over another folder. Do you remember whether you saw a fade out animation, or the icon suddenly disappeared without any delay ?

Yeah, there was a fade out animation.
And once they were all gone a refresh repopulated the app drawer with the app icons and all my folders had gone
