When adding apps, icons are coming over looking very fuzzy. Reverted to default icons thinking it was the icon pack and now all icons are fuzzy. Manually selecting the icon from customize menu applies at correct resolution.

Its not as bad if the default icon size is the same on the app drawer and the desktop, but they are still a little fuzzy and anti-aliased.

Usually blur will happen when the smooth option is on and there is no icon in the app for the device standard icon size. This might also happen if the desktop is zoomed a bit or the scale changed.
Did you increase the icon scale after having changed the icon ? If so this could also explain the poor quality because icons are stored using the current scale.

I do have the smooth option checked. It was better after resetting the scale to default, but it seems that icon packs set this to their own settings when applying them. I will test further with smooth option off and let you know. Thanks Pierre.

Ok. Quick update. :). To get the best/proper resolution:
Revert icons to default and remove smooth option.
Apply icon pack and check the scale %.
Revert back to default, then change the scale factor to the icon pack defaults.
Reapply icon pack.
All icons, overlays and masks are sized properly and nice and crisp.
