I’m running CM11 on my Galaxy S3. When I chose an icon pack from the CM theme chooser the icons in the LLX app drawer update but the icons on the desktop don’t. I have to add apps again for the new icons to show up.

Icons are updated when a package update notification is received. I guess that on CM such a notification is not sent for every app when changing the theme. You will probably have to pick again each icon manually.
