Bob Chernow I understood parts of the problem, and this is directly linked with the icon and background not updating. 3 symptoms for the same problem.
Have a look at your desktops and see if there is another folder that opens the same window. In theory this is not possible, but I have been able to see this on my device: two folders icons on two desktop open the same folder ! Your duplicate should be on the desktop number 1 (which was the main one before you switched to 3).
Do you remember how you created this folder, for instance did you create it on a desktop, then move it ? I really have no idea how to reproduce this.
Don’t remove any of these folders, because it will remove the content too. I am looking for a solution.
Ok, now I have a folder that was ok. I had 2 apps on it. The icon was a grid and showed the apps.. I removed the apps and the folder icon stayed as a grid showing the old apps. The I went to change background color of the folder and it took on the color of an older folder I Used to have on desktop 1. It even showed the old folders name. If I re add an app to it, it reverts the color and name.

I usually just long press on the desktop and add folder.
